Abstract

顎關節症患者, 男子 114名, 女子 262名, 總 376名을 對象으로 Oblique lateral transcranial
Projection으로 撮影한 後 tracing하여 顎關節症의 主症狀인 疼痛, 關節雜音, 開口障碍, 咀嚼
困難에 따라 顆頭의 量的變化를 調査하여 다음과 같은 結果를 얻었다.
1. 顎關節症患者總 376名中男子가 114名, 女子가 162名으로 女子에서 顯著히 많았으며,
男子에서 疼痛이 60名(52.6%), 關節雜音이 28名(24.6%), 開口障碍가 21名(18.4%), 咀嚼困難이
5名(4.4%)이였고. 女子에서 疼痛이 148名(56.5%), 關節雜音이 57名(21.8%), 開口障碍가 47名
(17.9%), 咀嚼困難이 10名(3.8%)으로 症狀은 男·女 모두에서 비슷한 比率을 보였다.
2. 正常群에서 男子는 W-W'가 右左側 각각 14.52㎜, 14.13㎜, O-H가 13.92㎜, 13.71㎜,
O-A가 8.91㎜, 9.03㎜, O-B는 8.67㎜, 8.78㎜였고, 女子에서 W-W'는 13.77㎜, 13.51㎜, O-H
는 13.42㎜, 13.35㎜, O-A는 8.92㎜, 9.01㎜, 그리고 O-B는 8.59㎜, 8.80㎜였다.
3. 顎關節症患者群에서 W-W'와 O-H가 正常群에 비해 뚜렷이 작았다. W-W'는 開口障
碍를 主訴로 하는 男女에서 가장 작았고(12.70㎜, 13.00㎜), O-H는 男·女 모두 疼痛에서 가
장 작았다. 關節雜音에선 O-H, O-A, O-B에서 正常群과 뚜렷한 差異를 보이지 않았으며,
O-B는咀嚼困難을 除外한 他症狀에서 正常狀과 별 差異가 없었다
症常에 따른 顆頭의 量的變化는 다음과 같다.
a) 疼痛을 主訴로 하는 경우, 男子의 W-W'는 右左側各各 13.80㎜, 12.80㎜, O-H는 13.10
㎜, 12.90㎜, O-A는 8.69㎜, 8.18㎜, O-B는 8.33㎜, 8.42㎜였으며, 女子에서는 W-W'가 13.01
㎜, 12.90㎜, O-H가 12.48㎜, 12.80㎜, O-A가 8.60㎜, 8.49㎜, O-B가 8.48㎜, 8.50㎜였다.
b) 關節雜音에선 男子가 W-W'는 13.70㎜, 13.10㎜, O-H는 13.90㎜, 13.10㎜, O-A는 8.81
㎜, 8.16㎜, O-B는 8.34㎜, 8.25㎜였고, 女子에서 W-W'는 13.10㎜, 12.50㎜, O-H는 13.30㎜,
12.91㎜, O-A는 8.95㎜, 8.49㎜, O-B는 8.23㎜, 8.70㎜였다.
c) 開口障碍는 男子에서 W-W'가 12.70㎜, 13.00㎜, O-H는 13.40㎜, 13.40㎜, O-A는 8.37
㎜, 8.48㎜, O-B는 8.33㎜, 8.62㎜였고, 女子에서는 W-W'가 13.00, 12.50㎜, O-H는 12.90,
13.10㎜, O-A는 8.49, 8.09㎜, O-B는 8.77, 8.01㎜였다.
d) 咀嚼困難의 경우 男子는 W-W'가 13.30㎜, 13.20㎜, O-H가 13.40, 12.60㎜, O-A는 8.26
㎜, 8.32㎜, O-B는 7.80㎜, 8.02㎜였고, 女子는 W-W'가 12.30㎜, 12.00㎜, O-H는 13.10㎜,
13.20㎜, O-A는 8.68㎜, 7.95㎜, O-B는 7.46㎜, 7.87㎜였다.
#초록#
The author obtained the oblique lateral transcranial radiograms from 376 patients (114
of male, and 262 of female) with temporomandibular joint arthrosis. After tracing each
film, the author analyzed the dimensional changes of the condylar heads with pain,
clicking, mouth opening limitation, and masticatory difficulty respectively, which were
the chief complaints of the TMJ arthrosis and compared these data with control group.
The results were as follows ;
1. There was a great predilection for occurrence of the TMJ arthrosis in female (262
cases) over male (114 cases). But there was no significant difference in ratio between
the sexes on each symptom.
In male, 60 patients (52.6%) had pain, 28 patients (2f .6%) had clicking,21 patients
(18.4%) had mouth opening limitation, and S patients (4.4%) had masticatory difficulty.
In female, 148 patients (56.5%) had pain, 57 patients (21.8%) had clicking, 47
patients(17.9%) had mouth opening limitation, and 10 patients (3.8%) had masticatory
difficulty
2. Examined controlled group, the author analyzed the dimension of W-W', O-H, 0-4,
O-B on the right side first, and left side second. In male, the dimension of W-W' was
14.52㎜, 14.13㎜ ; O-H was 13.92㎜, 13.71㎜ O-A was 8.91㎜, 9.03㎜ and O-B was 8.67
㎜, 8.78㎜.
In female, W-W' was 13.77㎜, 13.51㎜ ; O-H was 13.42㎜, 13.35㎜; O-A was 8.92㎜,
9.0l㎜ ; and O-B was 8.59㎜, 8.80㎜.
3. W-W' and O-H of the experimental group were distinctly lesser than the controlled
group. Male with mouth opening limitation show the least (12.70㎜, 13.00㎜) on W-W',
but both sexes with pain show the least on O-H.
There was no significant difference between experimental group with clicking and
controlled group on O-H,0-4, and O-B.
And also there was no difference on O-B in patients with every symptom except
masticatory difficulty.
The dimensional changes of the condylar heads with those symptoms were as follows ;
a) In male patients, complains of pain, W-W' of tar right and left was 13.80㎜, 12.80㎜ ;
O-H was 13.10㎜, 12.90㎜ ; O-A was 8.69㎜, 8.18㎜ ; O-B was 8.33㎜, 8.42㎜.
In female, W-W' was 13.01㎜, 12.90㎜ ; O-H was, 12.90㎜ ; O-H was 12.48㎜, 12.80㎜
; O-A was 8.60㎜, 8.49㎜: O-B was 8.48㎜, 8.50㎜
b) In male patients with clicking, W-W' was 13.70㎜, 13.10㎜ ; O-H was 13.90㎜, 13.10
㎜ ; O-A was 8.81㎜, 8.16㎜ O-B was 8.34㎜, 8.25㎜. In female, W-W' was 13.10㎜,
13.50㎜ ; O-H was 13.30㎜, 12.91㎜ ; O-A was 8.95㎜, 8.49㎜ ; O-B was 8.23㎜, 8.70㎜.
c) In male patients with mouth opening limitation, W-W' was 12.70㎜, 13.00㎜; O-H
was 13.40㎜, 13.40㎜ ; O-A was 8.37㎜, 8.48㎜ ; O-B was 8.33㎜, 8.62㎜.
In female, W-W' was 13.00㎜, 12.50㎜ ; O-H was 12.90㎜, 13.10㎜; O-A was 8.49㎜,
8.09㎜; O-B was 8.77㎜, 8.01㎜.
d) In male patients with masticatory difficulty, W-W' was 13.30㎜, 13.20㎜ ; O-H was
13.40㎜,12.60㎜ ; O-A was 8.26㎜, 8.32㎜ ; O-B was 7.80㎜, 8.20㎜.
In female, W-W' was 12.30㎜, 12.00㎜ ; O-H was 13.10㎜, 13.20㎜ ; O-A was 8.68㎜,
7.95㎜ ; O-B was 7.46㎜, 7.87㎜.
